ABSTRACT

Linggui Zhugan Tang(LGZG), a representative of warming spleen to resolve fluid retention, is widely used in various diseases caused by spleen deficiency and fluid retention (SDFR). Ancient doctors mostly focused on the theme of SDFR to analysis the composition of LGZG. Modern research has now found that most of the components of LGZG contain anti-inflammatory, anti-tumor and immune-modulating effects. At present, the pharmacological research of LGZG mainly involves clearance of β-amyloid (Aβ) and anti-neuroinflammation, regulation of lipid metabolism and insulin resistance, protection of myocardial cells and regulation of liquid metabolism as follow. LGZG can promote the clearance of Aβ by regulating the expression of Aβ transporters, including low-density lipoprotein receptor-associated protein-1 and terminal glycosylation product receptors, as well as reduce the inflammation of nerve cells by inhibiting the secretion of pro-inflammatory factors such as interleukin(IL) -1β,IL-6 and tumor necrosis factor-α(TNF-α) by microglia. The prescription can also affect the lipid metabolism and insulin resistance by regulating the levels of adiponectin, insulin and leptin, thus producing therapeutic effects on metabolic syndrome and non-alcoholic fatty liver disease. The protective mechanism of LGZG on myocardial cells mainly contains inhibition of the over-activation of nuclear transcription factor-κB(NF-κB) signaling pathway,up-regulation of Smad7 expression and down-regulation of Smad3 expression, regulation of inhibitor of nuclear factor kappa-B kinase (IKK)/NF-κB inhibitor (IκB)/NF-κB signaling pathway, inhibition of inflammatory factor production, and down-regulation of Caspase-3 and Caspase-8 expression. In addition,LGZG can also regulate aquaporins to affect water metabolism. And it also has significant antioxidant effects. As is known to all,the functional mechanism of traditional Chinese medicine (TCM) compound is closely related to the network regulation of TCM disease syndromes. Considered from this angle, it is helpful to carry out the "disease-syndrome-formula" research by means of network pharmacology characterized by "disease-target-drug", thus organically exploring the relationship between LGZG and SDFR from the microscopic perspective,and helping to explain the scientific connotation of SDFR and the biological mechanism of warming spleen to resolve fluid retention.

ABSTRACT

Objective Toinvestigatetheclinicalvalueofreadoutsegmentationoflongvariableecho-trainsdiffusion-weightedimaging (RESOLVE-DWI)inthediagnosisandstagingofthyroid-associatedophthalmopathy(TAO).Methods Atotalof30consecutivepatientswith TAOand30healthycontrols(HCs)whounderwentRESOLVE-DWIwereenrolledinourstudy.ADCvaluesofextraocularmuscles (superiorrectus,inferiorrectus,medialrectusandlateralrectus)were measuredandcomparedbetween TAOsand HCs,active TAOsandinactiveTAOs,orinactiveTAOsandHCs.ROCanalysiswasperformedtoevaluatethediagnosticvalueofsignificantparametersfor discriminatingactivefrominactiveTAOs.Results TheADCvaluesofallextraocularmusclesinTAOsweresignificantlyhigherthan thoseinHCs(P<0.05).Meanwhile,alltheextraocularmusclesinactiveTAOsshowedsignificantlyhigherADCvaluesthanthose ininactiveTAOs(P<0.05),exceptlateralrectus(P=0.267).WhilstnosignificantdifferenceswerefoundontheADCvaluesofall extraocularmusclesbetweeninactiveTAOsandHCs(P>0.05).ROCanalysisresultsindicatedthattheADCvalueofmedialrectus showedtheoptimalstagingefficacy(cutoffvalue,1.40×10-3 mm2/s;AUC,0.766;sensitivity,92.1%;specificity,59.1%).Conclusion RESOLVE-DWIanditsderivedADCvaluesofextraocularmusclescanassistinthediagnosisofTAO.TheADCvalueofmedial rectushastheoptimalefficacyontheevaluationofitsclinicalactivity.

ABSTRACT

[Objective]Investigating Professor WANG Peijuan's clinical experience in treating PCOS from studying the kidney. [Method] This paper mainly studied by arranging and researching the tutor's theory and experience about treating PCOS that based on the practice in clinic with the teacher as a self perception,the collection about the PCOS cases of tutor's clinical treatment and the analysis of national and international relative literature combined with self-awareness. [Result]Professor WANG thinks that PCOS is the disorder of qi and blood and the transformation failure of kidney-Tiangui-Chongren-uterus, which leads to phlegm and blood stasis, blocking in the uterus and making menstruation lose its inhere periodicity. The disease is of the kidney deficiency and phlegm stagnation syndrome.[Conclusion]Professor WANG holds that to use the method of tonifying kidney properly and attach importance to the three categories of etiologic factors will be effective to the treatment of PCOS. Her experience can provide the basis for diagnosis and treatment about the disease.

ABSTRACT

Hyperlipidemia, type 2 diabetes mellitus, nonalcoholic fatty liver and many other metabolic disorder are frequently co-existing in patients. In addition, these diseases are closely related in pathophysiological settings. However, increasing of the disease incidence, lacking of comprehensive prevention and control measurements against the key pathology point concomitant occurrence with the pattern of the single disease, single target therapy, that is leading therapeutic strategy for these metabolic disorders in the setting of Western medicine (WM). On the basis of the combination of the advantages of integrated Chinese medicine (CM) and WM, with unified understanding of such diseases, the new concept of glucolipid metabolic disease (GLMD) is introduced. In this new concept, disorders in glucose and lipid metabolism are recognized as the key trigger and major driving force for the progress of GLMD. The key points of pathology included dysfunction of neuronal-endocrine-immune system, insulin resistance, oxidative stress, inflammation and intestinal flora imbalance. In the core pathogenic perspective of CM, it can be explained as "Gan (Liver) Shi Shu Xie" (dysfunction of Gan in metabolism and emotion regulation) that will lead to the occurence/production of endogenous dampness and phlegm, blood stasis and turbid. This leads to the new concept of "Liver-based regulatory system for metabolic homeostasis" to be introduced further. The comprehensive prevention and control strategy "Tiao Gan Qi Shu Hua Zhuo" (modulating Gan, trigging key metabolic system to resolve pathogenic factors such as phlegm retention and dampness). Its representative formula Fufang Zhenzhu Tiaozhi Capsule () is innovated under such rationales. Comment for some commonly-used CM GLMD therapeutic drugs was presented. High-level evidence-based and epidemiological and mechanism studies should be carried out to further interpret and explain of the scientific connotation of GLMD.

ABSTRACT

Purpose] The article analyzed the rules of diagnosis and treatment of “menstruation stopped when not old yet”in the Fuqingzhunvke Section · Menstruation Chapter, which provides a new thought of diagnosing and treating for the treatment of premature ovarian failure. [Methods] Because of the characteristics of the provisions in the Fuqingzhunvke is consistent with the clinical symptoms of premature ovarian failure in the modern medicine. The article analyzed the cause, pathogenesis, principle-method-recipe-medicine and treatment to explore the treatment of premature ovarian failure. [Result] The characteristics of the provisions in the Fu Qingzhu Nvke are consistent with the clinical symptoms of premature ovarian failure in the modern medicine. Fushi thought that premature ovarian failure derived from heart, liver, spleen and kidney, treatment should reinforce kidney water;puts forward some new ideas for qi depression, nourishing liver to resolve depression method;pay attention to tonifying spleen, nourishing heart and tranquilizing mind; reminding relieving depression lies in strengthening the body resistance. [Conclusion] Through analyzing the etiology, pathogenesis, treatment of premature ovarian failure, to clarify the etiology, pathogenesis, treatment of premature ovarian failure. The study of classics of traditional Chinese medicine provides a new method for the treatment of premature ovarian failure. Making the past serve the present, the clinical curative effect will be improved.

@#Objective To explore the effect of Activating Blood to Resolve Stagnation on the expression of CD34 and vascular endothelial growth factor (VEGF) in rats with acute myocardial infarction. Methods 32 Sprague-Dawley rats were randomly divided into sham operation group (A, n=8), model group (B, n=8), Xuesaitong Injection + granulocyte colony- stimulating factor (G- CSF) group (C, n=8) and G-CSF group (D, n=8). Corresponding medicine was given to each group 3 hours after modeling, for 6 days. Pathomorphological changes were observed through HE staining, and the expression of CD34, VEGF and Ki-67 were observed through immunohistochemical staining. Results The expressions of CD34, VEGF and Ki-67 were higher in groups B, C and D than in group A (P<0.05), and were higher in group groups C and D than in group B (P<0.05). The expressions of CD34 and VEGF were higher in group C than in group D (P<0.05). However, there was no significant difference in the expression of Ki-67 between 2 groups (P>0.05). Conclusion The expression of CD34 and VEGF increases with Activating Blood to Resolve Stagnation method, which is superior to using G-CSF only. Activating Blood to Resolve Stagnation may play an important role in the treatment of acute myocardial infarction.

When it is cold winter in the northern China, the temperature of water is very low. Low tempera- ture is the main reason of the worse treatment of oil wastewater. In cold winter when the temperature is lowered to around 4?C, the effect of biological treatment of sewage is poor and hard to target the problem of reach a set standard of outing water. Use of microbiology to deal with oil pollution is one of the most effective, the most secure and the most thorough method. So the article from the microbiological point of view to study, isolated from nature world, screening to eight low-temperature grease which can grow under 5?C and degradation of sewage bacteria.

@# Diabetic peripheral neuropathy is one of the three main complications of diabetes. Its morbidity is up to 90%. Recent years, using Chinese medicine treating this disease is very popular in China, especially using activating blood to resolve stagnation to solve this problem. The author reviewed the progress of studies on treatment of diabetic peripheral neuropathy by activating blood to resolve stagnation, and put forward the future research direction.

ABSTRACT

AIM: To observe the difference of actions among three traditional compl ex prescriptions with the same composition but the different dosage, including X iaochengqi Decoction, Houpudahuang Decoction and Houpusanwu Decoction, so as to provide evidence for their different clinical applications. METHODS: A series of experiments were performed, including carbon p rop ellance in small intestine, black stool excretion, cough induced by ammonia liqu or and phenol red secretion from trachea. RESULTS: Three prescriptions all enhanced carbon propellance in sma ll intestine at the high dosage, while at the low dosage only Houpusanwu Decoct ion showed the significant effect. They all shortened the latency of black stool a nd increased their total amounts in 6 hours at the high dosage, while at the low dosage Xiaochengqi Decoction and Houpudahuang Decoction had the significant p urgative action. Houpudahuang Decoction remarkably prolonged cough latency, dec reased cough times and enhanced phenol red secretion from trachea, while other t wo prescriptions had no obvious effects. CONCLUSION: Such results showed that Xiaochengqi Decoction had the better purgative action, while Houpusanwu Decoction regulated Qi strongerly, a nd Houpudanghuang Decoction showed the marked actions of relieving cough and re solving phlegm.
